Cook County Health Foundation

Cook County Health Foundation is a 501(c)(3) nonprofit organization founded in 2011 and headquartered at 1603 Orrington Avenue in Evanston, Illinois. The Foundation is committed to expanding and improving healthcare for those with the fewest resources through support of Cook County Health, engaging in fundraising, education, and advocacy to build the system’s capacity to serve all Cook County residents. The Foundation invests in services, equipment, and innovative initiatives to address the health system’s and patients’ needs, while operating as an independent entity separate from the Cook County Health system itself.
