Fresh Food Farmacy – Shamokin

Fresh Food Farmacy – Shamokin is a Geisinger Health program launched in 2016 that combines fresh, healthy food with diabetes education to address food insecurity and chronic disease management. The program provides participants and their families with enough fresh, nutritious food to prepare ten healthy meals weekly, while offering clinical interventions including care management, diabetes education, and consultations with dietitians and pharmacists to help manage Type 2 diabetes and related health conditions.
