Church Health Services

Church Health Services, Inc. is a 501(c)(3) nonprofit healthcare organization located in Beaver Dam, Wisconsin that provides dental, mental health, and medical services to low-income, uninsured, and underinsured adults and children throughout South Central Wisconsin. The Christian-based organization offers comprehensive care including dental exams and cleanings, counseling services, behavioral health treatment, and walk-in medical clinics, with children’s counseling also available at school sites in Dodge and Fond du Lac Counties.
