hungercare coalition

The HungerCare Coalition is a program of Second Harvest Foodbank of Southern Wisconsin, established in January 2014 to educate healthcare professionals about food insecurity and connect patients with nutritional resources. The coalition partners with healthcare providers, hospitals, clinics, and community organizations throughout the 16-county service area to identify patients at risk of hunger and link them with emergency food assistance and nutrition programs. Through collaborative screening tools and partnerships with local health systems and food resources, HungerCare works to address the intersection of health and food security in Madison and surrounding communities.
